I, Pragati Tibdewal, Address: Tibdewal House, Dharamsala Lane, Sambalpur, declare that I have changed my name from Pragati Bansal to Pragati Tibdewal. Henceforth, I shall be known as Pragati Tibdewal for all purposes.
